Living Well For Longer

We at the PCN are thrilled to introduce our latest initiative, “Living Well For Longer.” This forward-thinking care program aims to enhance health outcomes while offering vital support to the most vulnerable members of our community.

The four main modules of this project are:

  1. At-home Structured Medication Reviews (SMR) - The provision of structured medication reviews to patients living with severe frailty who are housebound.
  2. Footsteps in Fibromyalgia - The Footsteps programme delivered by North Arden PCN aims to standardise the holistic care delivered to patients living with fibromyalgia by the wider multi-disciplinary approach to ensure equity and equality amongst patients, and to ensure that patients are supported in their individual long-term goals to live well with their condition. 
  3. Bereavement – The death of a loved one is a challenging time. We strive to assist our patients throughout the grieving process.
  4. Reaching out to those who don't reach out to us – We, as a PCN, would like to reach out to those in our community who don’t attend their surgery and see if we can find out why. Is it due to issues getting to the surgery, unwillingness to bother the doctor or something else? If patients want to see their doctor but find they are unable, we would like to do everything we can to help overcome these obstacles.

Through these comprehensive programs, we strive to prevent health deterioration and empower our vulnerable patients to live their best lives. Our focus is on enhancing their quality of life while ensuring they maintain their independence for as long as possible. By offering personalised care plans, access to essential resources, and ongoing support, we aim to address their unique needs and preferences. Additionally, we prioritise the importance of community engagement and collaboration with healthcare providers to create a supportive environment that fosters resilience and autonomy among our patients.
